SERVICES TO THE CITIZENS AND LGUs


ISSUANCE OF CERTIFIED TRUE COPIES OF RESOLUTIONS, ORDINANCES, JOURNAL OF PROCEEDINGS, MINUTES OF COMMITTEE HEARINGS AND AUDIO/VIDEO RECORDINGS

Requirements:
a) Properly filled-in request form
b) Payment of fee, Php50.00 per copy of a document

CLIENT STEPS REQUIRED ACTION TIME DURATION EMPLOYEE RESPONSIBLE
  • Get a document request form from the information desk and fill in the same, and submit it to the responsible employee
Provide the form, assist the client in filling in the form, receive the completed form, and issue order of payment attaching therewith the completed form 3 minutes Ms. Princess Gamboa
Evaluate whether the requested document is classified or not. Classified documents are not for public consumption; their issuance to the public is regulated. 1 minute Secretary to the SP
  • Proceed to the Provincial Treasurer’s Office and pay the document fee
Guide the client in going to the PTO. 1 minute Ms. Princess Gamboa
PTO receive the payment and issue official receipt 3 minutes PTO window personnel
  • Get back to the SP and present the official receipt to the responsible employee
Examine the veracity of the receipt and, if so, prepare the requested document and bring it to the Secretary to the SP for her signature. 5 minutes Ms. Princess Gamboa or Ms. Sally Obrador, depending upon the kinds of document being requested
Sign the prepared requested document 1 minute Secretary to the SP or her authorized person
Hand over to the client the certified copy of the document and have him/her sign the log book Ms. Princess Gamboa

REVIEW OF MUNICIPAL ORDINANCES AND OTHER MATTERS

  • Submit at least two (2) copies of ordinance or resolution for review; for appropriations ordinance, submit at least three (3) copies

Forward the document to the Legislative Division for inclusion in the agenda of the next session 3 minutes Ms. Princess Gamboa, Ms. Emilyn Calabio, Ms. Sally Obrador, or Secretary to the SP

ACTION ON ADMINISTRATIVE COMPLAINTS AGAINST MUNICIPAL OFFICIALS AND APPEALS AGAINST THE DECISION OF SANGGUNIANG BAYAN ON ADMINISTRATIVE COMPLAINTS

Requirements:

  1. At least fifteen (15) sets of complaint and its attachment
CLIENT/STEPS REQUIRED ACTION TIME DURATION EMPLOYEE RESPONSIBLE
  • Submit at least fifteen (15) sets of complaint/appeal and its attachments
Receive and check the completeness of the complaint/appeal. Return the client’s file copy affixing therewith the date, time, and signature of the responsible employee or receiving clerk 3 minutes Ms. Princess Gamboa
Forward the complaint/appeal to the Legislative/Quasi-Judicial Division 1 minute Ms. Princess Gamboa
Within seven (7) days from receipt of the complaint, require respondent to file an answer within fifteen (15) days from receipt of notice 11-32 days (Refer to Section 62 of the Local Government Code of 1991) Vice-Governor / Presiding Officer of the SP as a Quasi-Judicial Body

Note: In order for complaints to be set for hearing on the next session day, it must be submitted at least a day before the next session day.


ACCREDITATION OF CIVIL SOCIETY ORGANIZATIONS

Requirements:

  • Letter of Application;
  • Duly accomplished Application Form for Accreditation;
  • Board Resolution signifying intention for accreditation for the purpose of representation in the local special body;
  • Certificate of registration;
  • List of current officers and members;
  • For CSOs that have been in operation for at least one (1) year:
    • Minutes of the annual meetings of the immediately preceding year as certified by the organization’s board secretary indicating the date, location, attendees, and agenda;
    • Annual accomplishment report for the immediately preceding year; and
    • Financial statement of the preceding year signed by the executive officers of the organization indicating therein other information such as the sources of funds.

  • Receive the Certificate of Accreditation
Awarding of the certificate of accreditation to CSO; or transmittal of the certificate of accreditation Few minutes of a session day Vice-Governor / SP

ISSUANCE OF CERTIFICATIONS, SUCH AS FOR THE POSTING OR PUBLICATION OF OFFICIAL DOCUMENTS IN CONSPICUOUS PLACES WITHIN THE PREMISES OF THE PROVINCIAL GOVERNMENT

Requirements:

  1. At least three (3) copies of request and its attachments
CLIENT/STEPS REQUIRED ACTION TIME DURATION EMPLOYEE RESPONSIBLE
  • Submit at least three (3) copies of notice/order for posting or publication
Receive the document and return the client’s file copy affixing therewith the date, time, and signature of the responsible employee or the receiving clerk 3 minutes Ms. Princess Gamboa
Forward the document to the Secretary to the SP for proper action 2 minutes
Cause the posting of the document in conspicuous place/s within the premises of the Provincial Government for a period of time 2 minutes Secretary to the SP

  • Claim/receive the certificate of posting upon completion of the posting period

PROVISION OF ASSISTANCE TO INDIGENTS/CITIZENS

Requirements:

  1. At least two (2) copies of request and its attachment

  • Submit at least two (2) copies of request for assistance

  • Follow up the request

ACTION ON LOCAL INITIATIVE

Requirements:

  1. At least fifteen (15) sets of petition and its attachments

  • File a petition with the SP (adoption, enactment, repeal, or amendment of an ordinance)
Receive and forward the petition/proposition to the Legislative Division
Endorse the document to the Legislative Division

Act on the proposition Within 30 days SP
Transmit to the proponents the action of the SP Within 24 hours Sec. to the SP
  • If the SP has no favorable action within 30 days from presentation or filing of the petition, invoke and notify the SP of the power of initiative.
